  1. What do you use your computer to do?

  2. Are you happy with the way your system is working?

  3. Do you sometimes have trouble finding a program because you know what it does, but cannot remember what it is called?

  4. Do you have trouble finding websites you want, and cannot remember how you found them?

  5. Do you have trouble finding documents on your system after you have not used them for a while?

  6. Do you find yourself using slips of paper to write down numbers, names, addresses, and other information, even when your computer is turned on, because using paper is easier than using a computer?

  7. Do you avoid certain computer tasks because it takes too long to get all the things you need started and ready to go?

  8. When you solve computer problems, is it difficult to find the answer again?

  9. Do you have trouble translating computer steps or instructions into information that makes sense to you?

  10. Do you avoid buying new software because it is too difficult to learn how to use?

  11. Do you send a lot of email to friends or coworkers, and have trouble with finding or using email addresses?
